Proof of vaccination status for in office appointments


Clients who wish to attend in office appointments at MBHS must be fully vaccinated.
Fully vaccinated is defined as two vaccines (Pfizer or Moderna) or one shot (J&J). If you are eligible, you must also have a booster shot; if you are not eligible or have plans to receive the booster, then the reason will be noted with your filed vaccination card.

A copy of the front and back of your CDC Covid-19 Vaccination Record Card is necessary prior to your in-office appointment.

If you are not able to meet these requirements, you are welcome to attend Telehealth psychotherapy appointments via the Zoom platform only.
